Removal of plaque - A mystery pic with a difference?

The first person to supply the five missing words on this plaque and its location will gain 3 points! Feel free to guess the words and the location but I do need to see all the requirements are met in one answer to get the 3 points.

The end came in 1922, the last year when seiners were put in pay and a watch kept from Porthminster Baulking House. Only once since 1908 had the Pilchards come within 2 miles of the shore - but a train whistle frightened them off!! There you are thought you should know that ! read# ::15: ::6: :?:
